Bardet-Biedl Syndrome Treatment in Uganda

Scattered across nearly every cell in the human body are microscopic, hair-like projections called cilia, and when the genes that build and maintain them break down, the effects ripple across multiple organs at once. That’s the underlying story behind a family of conditions called ciliopathies, and Bardet-Biedl Syndrome (BBS) is one of its better-known members. Vision loss shows up reliably in BBS, but it’s only one piece of a larger systemic picture that families in Uganda are usually already managing through their local physicians before they ever contact us.
Given how widely BBS reaches across the body, we keep our own role tightly defined: existing medical records are reviewed ahead of time to understand the full systemic context, and the in-person visit itself is reserved for the retinal testing and eye-care planning that depends on equipment most general clinics simply don’t have on hand.

    Ocular Symptoms

    The eye involvement in BBS follows a rod-cone trajectory — night vision trouble showing up in early-to-mid childhood, a visual field that keeps closing in through the teenage years, and central vision that’s often affected a bit sooner than it would be in RP without an associated syndrome. Outside the eyes, BBS is frequently accompanied by extra fingers or toes, weight gain that starts in childhood, and kidney function that isn’t quite what it should be — all of which stay under the care of other specialists.

    Underlying Causes

    More than twenty genes have been tied to BBS so far, each one interfering with how primary cilia — present on almost every cell type — normally do their job, which explains why the condition touches so many organ systems simultaneously. Autosomal recessive inheritance accounts for the vast majority of cases.

    Diagnosis for Ugandan Patients

    We run electroretinography, OCT imaging, and visual field testing as our core retinal assessment, adding fundus photography to keep track of how things shift from one visit to the next. BBS-related vision decline can outpace non-syndromic RP in speed, so we generally set up a baseline exam followed by a structured monitoring schedule rather than treating it as a single one-time check.

    Treatment Approach in India

    For eligible patients, we consider regenerative stem cell therapy as a supportive measure aimed squarely at the retinal findings — not at the syndrome as a whole. Given that BBS-related retinal disease often moves a little faster than isolated RP, we generally recommend a tighter follow-up schedule after a patient returns home to Uganda, adjusted to match how quickly that individual’s condition is changing.
